Welcome to this downloads page. If you have come here it means you have enroled on a diving course, congratulations! All the forms below are PDF files which you need to print off, complete and hand to your instructor. A brief summary is attached to each file.

You will also need 2 passport photos for each course, and these can be mailed to the club if you have them electonically.

	Medical Forms (Reduced Size)	A critical form to ensure you are fit for diving. Answer all the questions yes or no. If you answer yes to ANY question download the next file and get medical clearance from your GP or the visiting Diving Doctor at Extreme Marine
	Medical Form (Full Size)	Use these forms to take to a GP, they contain detailed medical information.
	Continuing Education Administration form	Now used for all non pro courses beyond Open water including specialities, one forms lasts 12 months regardless of the number of courses taken.
	Liability Release	This spells out the risks and the circumstances where you dive at your own risk
	Safe Diving Practices	This is where you are infomed of the necessary safety practices.
	Bubblemakers Form	Release form for the 8-10 year old.
	Youth Diving Form	Release for the 10-15 year old.
	Membership form Word 2007 format	Same as below, just in Word
	Membership Application Form	Join The Club. Only £60 for adults, £25 for juniors or £140 for a family. You can use the pool every week for this, nothing else to pay.
